In order
to complete your 2008 on-line registration, you must do the following:
1. Complete the registration form below.
2. After
filling out the form AND before submitting it, please
PRINT a copy of the form for your
own records.
3. After Submitting your Registration, pay for your registration by
selecting the items on the Pay-Pal page that you
submitted on the Registration Form.

and mail it with your payment.

Family Event Rates
LOMT Family Events at Camp Lone Star are priced with an adult rate (over age 18), a youth rate (age 5-18) and
children under age 5 are NO CHARGE. For planning purposes we need to know the names and ages of all
participants attending, whether they are charged or not. Please complete the bottom portion of the registration
form and list all camp attendees in your family.
If a cancellation request is made two (2) weeks or more before the check-in date of the camp session registered
for, the registration and other fees minus $100 per weeklong session adult ($50.00 per weekend session adult)
and $25.00 per paying child may be refunded or transferred to another session.
If the cancellation request is made less than two (2) weeks before the check-in date of the camp session registered for, ONLY IF a replacement family is secured will the registration and other fees minus $100 per weeklong session adult ($50.00 per weekend session adult) and $25.00 per paying child may be refunded or transferred to another session.
For cancellation requests made less than twenty-four (24) hours before check-in, there will be no refund or transfer of registration fees.
The Management Committee may make exceptions for medical reasons when provided substantive documentation.
